Enrollment Process for 
Supervised Parenting Time or Safe Exchanges:

DO NOT complete this form if you are trying to register for a parenting course. 

1. Complete the Intake Form below. 

2. Prepay the $40 Intake Fee:

3. Once we recieve the form and the intake fee,
we will reach out by email to schedule your intake appintment. 
 
4. Gather your documents-
Valid ID, Court Order, PPO, CPS Reports, etc. It is essential to have the current court order OR written agreement regarding supervised parenting time prior to the intake appointment.

5. Attend your intake appointment. We will discuss the family background and our policies and procedures. Please do not bring children to this appointment.
